It will help you to select an appropriate plan for yourself and your family. It also helps you to understand the required underwriting process to expedite your application for approval for the coverage that you applied for.

What is International Major Medical Plan?
If you apply for one of the following international major medical plans such as PLATINUM PLAN, ICE, PREMIER, GLOBAL MEDICAL INSURANCE, GLOBAL BASIC INSURANCE, and RESIDE PRIME WORLDWIDE MEDICAL PLAN on our website, you are applying for a major medical plan. These major international medical plans provide $US5,000,000 lifetime benefits with comprehensive medical coverage such as doctor visits, maternity, well child care, surgical consultant, chiropractors, organ transplants, hospital room and board, including worldwide Emergency Medical Evacuation, Repatriation of Remains, and emergency. Am I Eligible for International Major Medical Plan on Sunburst Website?
Our international major medical plans are available to citizens of all countries of the world who are at least age 14 days and not older than age 70.

Please contact us at 877-211-3654 or info@worldwidemedicalplans.com before submitting your application if you live or have a mailing address in Florida.

1) If you are a U.S. citizen, you must reside outside the U.S., or plan to depart the U.S. within 30 days of the effective date. If you are a U.S. citizen, you must also reside outside the U.S. for at least 6 months within each 12 months of Certificate period.

2) If you are under 65 and a green card holder or permanent resident of the United States, you must reside outside the U.S., or plan to depart the US within 30 days of the effective date. You must also reside outside the U.S. for at least 6 months within each 12 months of Certificate period. 3) Citizens of other countries may reside anywhere, including their country of citizenship.

Am I Eligible for Optional Term Life?
Optional Term Life Insurance is not available to residents of the U.S., regardless of your citizenship.

How to complete an application?
You must answer very question completely and truthfully. Failure to answer very question will delay your application from being approved.
 
1)
If you are a U.S. citizen or a green card holder or permanent resident of the U.S., you must provide resident address outside of the United States, date of departure from the U.S., and length or residence outside of the U.S.

2) Provide complete details of medical condition including: treatment dates, name, address and telephone number of the treating physician, diagnosis, prognosis, and present course of treatment with complete names of medication.

3) The underwriters may request additional information. Be prepare to submit medical records promptly.

4) Be sure to print out your completed application for your records.

SPECIAL NOTE: After submitting your application, please check your email daily and look for e-mail from Sunburst Worldwide Insurance Services with your name on the subject line. We will send you the underwriter's request for additional information as well as to inform you the status of your application via e-mail. Please response to our e-mail promptly via e-mail as we need to have the requested additional information in writing.
